Please note that we have made a major change to our pre-camp packet of forms for 2012.
In addition to the required medical, travel and profile forms, several other forms and informational pages are included in the supplemental pre-camp packet, available in pdf format here.
Request a hard copy of the Supplemental Pre-Camp Packet or any requuired forms by emailing the office.
The Supplemental Pre-Camp Packet contains the following information:
- General Information
- Statement of Camp Policies
- Camper Contract** (must be completed prior to attendance)
- Money at Camp
- Trips and Options
- White Water Release Form
- Credit Card Authorization
- Packing List
